By scheduling an appointment with Restorative Health of Colorado, you acknowledge and agree to the policies outlined below.
Treatment recommendations are based on the information provided during your consultation, evaluation, and medical history. Individual results vary and depend on factors including medical history, adherence to treatment recommendations, lifestyle factors, and biological response. No guarantees regarding treatment outcomes are expressed or implied.
Patients are responsible for providing complete and accurate medical history, medication information, and updates regarding any changes in their health status. Failure to disclose relevant medical information may affect treatment eligibility, safety, and outcomes.
Patients are financially responsible for all services rendered, including any laboratory testing, medications, supplements, or products purchased through the clinic unless otherwise specified in writing.
Payment is due at the time services are rendered unless alternative arrangements have been made in advance. Pricing and service offerings may be updated periodically. Current pricing will be reviewed prior to treatment and is available upon request.
Restorative Health of Colorado reserves the right to refuse service when treatment is deemed medically inappropriate, unsafe, or outside the provider’s scope of practice.
By booking an appointment, you consent to receive appointment reminders and clinic communications via phone, text message, and/or email.
At Restorative Health of Colorado, we value both your time and the time reserved specifically for your appointment. To help us provide exceptional care to all patients, we kindly request at least 24 hours’ notice if you need to cancel or reschedule your appointment.
Appointments canceled with less than 24 hours’ notice, missed appointments, or no-shows may be subject to the following fees:
• Hormone Therapy Consultation: $100
• Weight Loss Consultation: $100
• Botox/Xeomin Appointment: $50
• Wellness Injection Appointment: $50
• IV Therapy Appointment: $50
These fees help offset the time reserved exclusively for your appointment that cannot be offered to another patient on short notice.
By scheduling an appointment, you authorize Restorative Health of Colorado to charge the applicable cancellation or no-show fee to the payment method on file if sufficient notice is not provided.
Patients arriving more than 15 minutes late may be asked to reschedule their appointment to avoid disrupting the schedule of other patients. If the appointment cannot be accommodated, the applicable late cancellation fee may apply.
Patients with a history of missed appointments, no-shows, or repeated late cancellations may be required to maintain a credit card on file or prepay for future appointments.
Payments for services rendered are non-refundable. Prepaid packages, memberships, promotional purchases, and treatment packages are subject to their individual terms and conditions. Any refunds, exchanges, or credits are provided solely at the discretion of the clinic and in accordance with applicable laws.
Our goal is to provide a safe, welcoming, and professional environment for all patients and staff.
• Please arrive 10–15 minutes before your scheduled appointment if paperwork or updates are needed.
• Patients receiving aesthetic or wellness treatments should carefully follow all pre-treatment and post-treatment instructions to help ensure optimal results and reduce the risk of complications.
• Clinical photographs may be taken for medical documentation, treatment planning, and progress tracking. Separate written consent will always be obtained before any photographs are used for marketing, advertising, educational, or promotional purposes.
• For the safety and comfort of all patients, we kindly ask that children not be left unattended in the clinic during appointments.
• To maintain a relaxing and professional atmosphere, we ask that cell phones be silenced during appointments whenever possible.
